Click below to complete or edit your medical history for our clinical screeners.  This helps us ensure a safer procedure for you.  We ask that the form be completed no later than 5 days before your scheduled procedure when possible.  Completing the passport should take less than 30 minutes. 

St. Clair Health’s Surgical Passport makes the surgery process easier for patients

The Surgical Passport, supported by One Medical Passport, is an online form that allows patients to provide complete, accurate medical information to their healthcare team. 

The website allows users to enter their information at any time, from anywhere. You will be asked to create your own username & password for future use when returning to the website. 

First Time Users:  Once you have clicked above to go to One Medical Passport’s website, click the green “Register” button to create a Surgical Passport. Enter your demographic information and create a username & password. Answer the questions on each page, then click ‘Save & Continue.’ Once complete, click ‘Finish’ to submit your information to us. The information you provide is kept on a secure site, is password protected by you, and is never shared with anyone other than your healthcare team.

Returning Users:  If you’ve previously created a One Medical Passport Account and are having another procedure at St. Clair Health, you will only need to verify and/or update your information. Enter your original username and password in the ‘Welcome Back’ area and click ‘Sign In.’ 

Why should patients complete a Surgical Passport?

Surgical Passport provides patients with automated reminders, instructions and educational information customized to their specific surgical procedure, their physician’s instructions, the patient’s medical history and medication. For example, messages or videos can include information regarding stopping/starting medication and eating/drinking before your procedure. These messages are specific to the patient because it is tightly integrated with the patient’s online pre-admissions information. 

Information to have available while completing the form:

  • Health insurance information
  • Names, addresses and phone numbers of your physicians
  • A list of all medications you are taking, their dosage and frequency
  • A list of all surgical procedures you’ve ever had
  • The name of the surgeon who will be performing your procedure

Is my private health information safe?

Yes, your information is stored on servers at a secure webhosting site and will only be released to your medical team. Your information is never released to anyone other than your medical team.
